DECLARATION OF CONSENT TO TREATMENT
OF PERSONAL, IDENTIFYING, SENSITIVE AND LEGAL DATA
Legislative Decree no. 196 30 June 2OO3

WHEREAS

1.-That in accordance with article 23 ("consent") of Legislative Decree no. 196/03 the processing of personal data by private parties is allowed only with the express consent of the concerned person. The consent is freely provided to an identified and documented treatment , which is preceded by the informative note with refer to the article 13 of Legislative Decree no. 196/03.

2.-That, in accordance with article 23 ("consent") of Legislative Decree no. 196/03, if the treatment involves sensitive data too, or only, the consent must be expressed in writing, except in the cases referred to the article 26 (4), letter c), the content of which I declare to know.

3.-That, in accordance with the article 13 ("privacy policy") of Legislative Decree No. 196/03, the content of which I declare to know.

4.-The data will be processed to guarantee security and confidentiality. Automated tools will be used to enable the storage, management and transmission of the same ones.

5.-The data and required documents in refer to ongoing, established or phased out assignments will be kept in storage for the period of 10 years

 

 

6.-The data processed by automated tools will be deleted to the exhaustion of the conferred mandate, except those relevant and not excessive in relation to subsequent assignments provided by the same customer ("subject").

7.-Please note that the subject has the right, in refer to article 52 of Legislative Decree no. 196/03, to ask – in the indicated manner and terms – the omission, for legitimate reasons, of general information and other identifying data in the case of diffusion of any judgment or other legal action.

8.-If this authorization to the processing of personal, identifying, sensitive and legal data should be signed the contained information will be valid for the positions which were opened before 01.01.2004 too.

SPONTANEOUSLY I DECLARE

to authorise, in accordance with the above indicated notes, and more generally in accordance with Legislative Decree no 196/03, the treatment of my personal data, including the identifying, sensitive and legal ones, specifying that for any judicial phase a special mandate in the forms of law will be issued
